Donation Form ANNUAL GENERAL MEMBERSHIP FEE: The $75 General Membership (Family) fee is an annual fee effective calendar year, January through December. This fee is assessed to each current family membership to help cover the costs of insurance, equipment, field maintenance, gym space rental, and other administrative costs to run all of our sports programs. YOU WILL BE ASSESSED THIS FEE WHEN YOU REGISTER ONLINE FOR THE FIRST SPORT PLAYED IN THE CALENDAR YEAR. PLEASE NOTE: The Family fee is predicated on the combination of “last name” and “primary residence home phone number” you enter on the form. You need to enter the SAME phone number every time you register a player or the system will not recognize that this is the same family and will assess the fee again.
